At Ishacare, we provide both Support Coordination (Level 2) and Specialist Support Coordination (Level 3) to support NDIS participants in implementing and managing their plans effectively.
We understand that navigating the NDIS can sometimes feel overwhelming - particularly when coordinating multiple services, understanding plan funding, or managing more complex situations. Our role is to work closely with you to ensure your supports are well coordinated, responsive, and aligned with your goals.
Support Coordination (Level 2) focuses on helping participants understand their NDIS plan, connect with appropriate service providers, and build the skills needed to manage supports more independently over time.
Specialist Support Coordination (Level 3) provides a higher level of support for participants with more complex needs who require additional guidance to navigate challenges and coordinate multiple services.
What is Level 3 Specialist Support Coordination?
Level 3 Specialist Support Coordination is designed for participants who:
- Have complex support needs
- Are experiencing crisis or high-risk situations
- Require specialist intervention
- Need intensive coordination across multiple services
- Face barriers to accessing appropriate supports
This level of coordination provides a more hands-on approach to managing risk, stabilising supports, and building long-term capacity so participants can achieve their goals.
